Evenstate Wellness is an independent insurance practice operated by Carrie J. Perna, a licensed insurance producer (NPN #22195564). We help individuals, families, and small businesses compare and select health, dental, vision, and life insurance coverage. As an independent producer, we are not obligated to any single insurance carrier and may present options from multiple carriers.

We are currently licensed to sell insurance in the following states: AL, AR, AK, AZ, CA, CO, FL, LA, ME, MI, NC, NJ, OH, OK, SC, SD, TN, TX, UT, VA, WV. We do not offer or sell insurance products to residents of states in which we are not licensed. Please confirm your state of residence is on this list, or contact us to verify current licensing before relying on any information from this Site.

The content on this Site — including descriptions of health, dental, vision, and life insurance products — is provided for general informational and educational purposes only. It is not:

• A quote, offer, binder, or guarantee of coverage;
